Personal medical book (part 2)

If an organization deals with a licensed and accredited healthcare institution when issuing medical books for its employees, all costs associated with paying for medical examinations of employees, as well as hygienic training and certification with the issuance of personal medical books, can be attributed to the expenses of the organization.

If there is no license and / or accreditation, then such expenses at the first fiscal check will be attributed to profit with all the ensuing consequences (penalties for non-payment of taxes, penalties, etc.).

In accordance with the order of 05.20.05 N402, upon dismissal and transfer to another place of work, the personal medical book remains with the owner and is presented at the place of new work.

A personal medical book is a document of strict accountability and is not subject to free sale. Only the Hygiene and Epidemiology Centers in the constituent entities of the Russian Federation and their branches are entitled to issue LMK.

In connection with the reorganization of the state sanitary and epidemiological service, the changes also affected the LMK form. The Federal Service for Supervision of Consumer Rights Protection and Human Well-Being approved the order of May 20, 05 N402 “On personal medical book and sanitary passport” instead of the previous order of the Ministry of Health of Russia dated April 14, 00 N 122 “On personal medical book and sanitary passport for transport means for transportation of food products. ”

The changes affected mainly the appearance of the document, in particular, the word “Sanitary Inspection” was replaced by the word “Rospotrebnadzor”. In addition, the horizontal orientation of the 2nd and 3rd pages has been adopted – this is done for the convenience of automatically filling out personal medical books (printing on a printer).

LMK are printed in the Moscow printing house GOZNAKA (“The First Printing Yard”). Only enterprises licensed by the Ministry of Finance of Russia are entitled to supply these products to places.

The cover and pages of LMK are made of special materials and using technology that meets the requirements for durable documents (passport, military ID, etc.).

The LMK size is 97 × 135 mm, the corners are rounded, the color of the cover is dark blue. Each personal medical book has a seven-digit number. For printing, strict accounting paper with watermarks is used. On the last page – a mandatory record of the manufacturer. Strict accounting even extends to the security thread used to staple forms.

In order to protect LMK from counterfeiting, it additionally contains:

in the center of the cover is an embossed inscription made in golden color – “personal medical book”;
on each page of the book and the inside of the cover there is a blue grid in the form of half rings with “hairs”, with the inscription in the center – “personal medical book”. A fragment of the grid is represented by stylized letters “M” and “D” (medical documents);
on each page at the top center is the section LMK. Duplication of sections 5, 6, 7, 8, 9, 10, and two or more pages is allowed. Additionally, the authenticity of a personal medical book can be established by the following criteria:
round stamp “For personal medical books” Federal State Health Institution “Center for Hygiene and Epidemiology in the City _______” or its branch, which issued a book certifying the photograph of the owner;
holograms, one of which certifies the photo of the owner, the other is the certification result, which is entered on pages 29-30;
registration number by which the book is checked for authenticity (affixed in the lower right corner of page 2).
It is very simple to distinguish fake medical books from real ones: they do not have watermarks, are not protected by holographic stamps, the cover and sheets of such books are often of different colors (blue and green), sewn with multi-colored threads, fastened with metal staples, with or without numbers.

The numbers of all personal medical books issued by the Center for Hygiene and Epidemiology Federal State Health Institution are entered in the Unified State Register, which easily verifies the authenticity of the verified LMC. Specialists of Rospotrebnadzor, as well as law enforcement officers, have the right to control the availability of personal medical books for employees.

The responsibility for the admission to work of persons who do not have fully executed LMK, in accordance with the law, lies with the employer.

If a fake medical book is discovered (and this is often the case now), the responsibility lies with both the specific employee and the employer.
